It is a requirement of the EYFS to regularly communicate between providers if a child is attending more than one setting. This template form is ready for you to input your knowledge of the child within. It is very quick and easy to complete with each section being self-explanatory but there is a handy guide on the first page to aid your completion. The form can easily be sent via email or completed on paper and passed between settings. It supports you to ensure individual children’s needs are met and provide some consistency to the child. Because this form is emailed to you, you have unlimited use of it for the one price you pay.
