Description
It is a requirement of the EYFS to share information between settings and best practice has seen this communication extended to include transition reports when a child leaves for school. Whatever transition a child is facing, here is a template form ready to be populated. Each section of the areas of learning and development are prefilled with hints and suggestions (not shown in the picture) of what to include. There is a handy guide on the first page to aid you when completing a transition report. The prefilled text can be easily deleted as you complete the form. Because this form is emailed to you, you have unlimited use of it for the one price you pay, and it can be easily adapted to suit any transition.
